A permit to work is the mechanism that stops two crews from unknowingly killing each other. One team isolates a line for maintenance; another team, working a different job order, restores power to the same circuit because nothing told them not to. The permit exists to make that collision impossible, and it only works when it is treated as a control rather than a form.

The Failure Mode Is Almost Always Coordination

Serious incidents on multi-contractor sites rarely involve one person doing something obviously reckless. They involve two reasonable activities that were safe in isolation and lethal in combination: hot work near a tank someone else was draining, an excavation over a service nobody mapped, a confined space entry beneath scaffolding being dismantled. Nobody was careless. Nobody was talking to each other either.

Bringing in a contractor transfers the work, not the responsibility. If an incident happens on your site, the questions asked afterwards are about your selection process, your induction, your supervision and your permit system. What a Working Permit System Controls

  • Scope and duration — one specific task, one location, one shift, with an explicit expiry rather than an open end.
  • Isolation verification — the issuer confirms isolation physically, not by asking whether it was done.
  • Simultaneous operations — a check against every other active permit on site before this one is issued.
  • Named competent people — an issuer and an acceptor who both understand the hazard, not whoever is available.
  • Formal closure — the permit is handed back, the area confirmed safe, and isolations removed deliberately.

Induction Is Not a Signature Sheet

Most contractor inductions consist of a slide deck and a signature. What actually changes behaviour is much smaller and more specific: where the assembly point is, who to call, what the alarm sounds like, which areas need a permit, and who has authority to stop work. A short induction delivered in the worker's own language, covering those five things, outperforms a long one delivered in a language half the crew is guessing at.

The test of a permit system is simple: pick any active permit at random and walk to the job. If what you find matches what the permit describes, the system is real. If it does not, the paperwork is documenting an intention rather than controlling a hazard.
